요추5번-천추1번 간 추간공 협착증 및 베르톨로티 증후군에 대한 양방향 내시경 수술
요추5번과 천추1번 디스크가 발생할 때 간혹 추간공 협착증을 유발함과 동시에 요추5번의 횡돌기와 골반뼈가 붙어버리는 이행성 척추가 동반된 경우가 있습니다.
이 경우 해당부위에 골반 통증이 유발되는 경우가 있는데 이를 베르톨로티 증후군이라고도 합니다.
이행성 척추가 무게를 분산시켜 해당 부위에 무게가 덜실리게 하는 경우도 있지만 오히려 붙어버린 가관절 부위에 통증이 생기고 척추와 골반이 붙어버리면서 부분적인 측만증을 유발하여 그 위쪽인 요추 4-5번간 디스크 문제를 추가적으로 일으키기도 합니다.
우측 엉덩이 부터 발까지 통증과 저림증상이 있으신데 다른곳에서 시술을 받아도 호전이 없어 방송을 보고 저에게 오셨다고 합니다.
하여 해당 부위에 양방향 내시경으로 추간공 협착증 제거 수술을 했습니다.
우선은 내시경을 요추 5번 횡돌기 부근에 거치시킨 후 관절간부와 상관절돌기를 찾습니다
그 부위를 드릴로 갈다보면 디스크가 마모가 많이되어 상관절돌기와 관절간부가 거의 맞닿아 있는 것을 볼 수 있습니다.
상관절돌기와 횡돌기, 관절간부 사이를 계속 파고 들다보면 추간공 인대가 나오고
추간공 인대를 떼어내면 요추5번 신경뿌리가 나옵니다.
공간이 너무 없어 치즐로 기둥뼈를 일부 쳐냈습니다.
이후 신경뿌리가 자유로워지며 요추 5번-천추 1번 디스크를 볼 수 있었고 디스크는 절제하지 않고 겉면만 태워 부피를 줄였습니다.
요추5번 횡돌기와 골반뼈가 붙은 가관절부위는 드릴로 갈아서 서로 떼어내줍니다.
솔직히 말하면 환자는 수술 후 2일간 통증이 지속되었습니다. 하지만 핏줄을 뽑고나서 급격히 통증 호전을 보였고 퇴원할 때는 많이 좋아지신 상태로 퇴원하였습니다.
얼마전에 1달 추적관찰을 했는데 많이 좋아져서 잘 걸어다니게 되니 좋다고 하십니다.
수술 2일 후에 좋아지신게 요추 5번이 움직임이 자유로워 지면서 부분적 측만증이 좋아져서 그런건지 아니면 핏줄을 뽑고 나서 좋아진건지는 모르겠지만 1달째 촬영한 xray에서는 부분적 측만증은 점차 호전추세를 보였습니다.
Biportal Endoscopic Surgery for Foraminal Stenosis Between Lumbar 5 and Sacral 1 Vertebrae and Bertolotti's Syndrome
When a disc issue arises between the Lumbar 5 (L5) and Sacral 1 (S1) vertebrae, it sometimes leads to foraminal stenosis, while concurrently, a transitional vertebra occurs where the transverse process of L5 fuses with the pelvic bone.
This condition can cause pelvic pain, which is also known as Bertolotti's Syndrome.
Although the transitional vertebra can help in distributing the weight, thus reducing the load on the area, the fusion of the spine and pelvis can induce pain in the fused sacroiliac joint area and partial scoliosis, further leading to additional disc issues between Lumbar 4 (L4) and L5.
The patient came to me with pain and tingling symptoms from the right hip down to the foot. He sought treatment after not seeing improvement from previous procedures elsewhere.
Therefore, I performed a biportal endoscopic surgery to remove the foraminal stenosis at the affected area.
First, the endoscope was positioned near the transverse process of L5, followed by locating the facet joints and the articular processes.
Drilling in this area revealed significant disc wear, with the articular processes and facet joints almost touching each other.
Continuing to drill between the articular processes, transverse process, and facet joints eventually exposed the foraminal ligament.
Removing the foraminal ligament revealed the nerve root of L5.
Due to limited space, a chisel was used to remove part of the vertebral column.
This allowed the nerve root to become free, and the disc between L5 and S1 became visible. The disc was not removed, but its surface was cauterized to reduce its size.
The fused area of the L5 transverse process and the pelvic bone in the sacroiliac joint was drilled to separate them.
To be honest, the patient continued to experience pain for two days post-surgery. However, after removing the drain bag, there was a significant improvement in pain, and he was discharged in a much better condition.
A one-month follow-up revealed considerable improvement, with the patient walking well.
It's unclear whether the improvement two days post-surgery was due to the increased freedom of movement in L5 leading to reduced partial scoliosis, or the removal of drain bag, but the X-ray taken in the first month showed a gradual improvement in the partial scoliosis.
