척추 유합술 수술 후 핏줄을 사용하는 것과 사용하지 않는 것의 차이에 대한 연구 결과가 실렸습니다.
핏줄은 한 번쯤 수술을 받아보신 분들은 아실 수도 있는데요
수술 부위에 관을 넣어두고 나와 수술 부위에 피가 고이지 않게 하는 배액관입니다.
이번 연구는 칠레에서 진행되었고 전향적 랜덤 연구로 진행이 되었는데요
결과로는
척추 유합술 수술 후 핏줄을 삽입해 놓은 환자들이나 혹은 핏줄을 삽입하지 않은 환자군을 비교했을 때
상처 감염률이나, 혈종이 고일 위험도 등 합병증 발생에는 아무런 통계적 차이가 없었고
그리고 핏줄을 삽입하지 않은 환자들이 병원에 입원해 있는 기간이 짧았다고 합니다.
또한 정상체중과 비만 환자들을 나누어서 비교를 했을 때도 합병증에 차이가 없었으며 핏줄을 삽입하는 것이 큰 이점이 없었다고 합니다.
그러면서 저자들은 핏줄의 사용을 권장하지 않는 다는 것을 결론으로 내렸지만
혈종이 고일만한 높은 위험도를 가진 환자에서는 역할을 할 수 있다고 마지막에 언급을 하였네요.
하지만 제 생각에 수술하는 사람의 입장에서는 오히려 감염률에 큰 차이가 없다고 한다면 만에 하나 발생할 수 있는 혈종 가능성에 대비하여 시간을 몇 분 더 들여서라도 핏줄을 넣는 것을 고려할 수도 있겠습니다.
In the April issue of The Spine Journal, a famous spine academic journal, there is a research result on the difference between using and not using a drain after spinal fusion surgery.
A drain, which some of you may know if you've had surgery before, is a tube placed in the surgical area to prevent blood from pooling at the site.
This study was conducted in Chile and was carried out as a prospective randomized trial.
The results showed that there was no statistical difference in wound infection rate, hematoma risk, or complications when comparing patients with and without a drain after spine fusion surgery.
However, patients without a drain had a shorter hospital stay.
There was also no difference in complications when comparing normal weight and obese patients, and it was found that inserting a drain did not provide significant benefits.
The authors concluded that they do not recommend the use of drains, but mentioned at the end that they can play a role in patients with a high risk of hematoma formation.
However, from the perspective of the person performing the surgery, if they say there is not much difference in the infection rate, they may consider taking a few extra minutes to insert a blood vessel to prepare for the possibility of a rare occurrence of hematoma formation.
