척추 수술 후 감염이 발생 했을 때 대처 방법이 실패하는 원인에 대한 분석을 하였습니다.
이 연구는 미국 예일대학교에서 후향적 연구로 진행이 되었습니다.
척추 수술 후 감염률은 앞선 연구들에서 1~3.1% 정도로 보고가 되고 있습니다.
그리고 만약 척추 수술 후 감염이 발생 할 경우 보통의 대처 방법은 상처를 다시 열고
상처 부위를 세척 한 후 배액관을 넣어놓고 항생제 정맥 주사를 사용하는 방법이 일반적인데요
연구자들은 한번의 세척 조치(I&D)로 해결이 된 경우가
72.7% 였고
한번의 세척 조치(I&D)로 해결이 안되었던 경우가
27.3%에 달했다고 합니다.
한번의 세척 조치(I&D)로 감염 해결이 실패한 중요한 요인은
패혈증이 동반된 환자들이었는데요
패혈증이란 혈액 검사에서 균이 검출되는 경우로
패혈증이 있는 경우 한번의 세척 조치(I&D)로 수술 부위 감염이 해결이 안된 경우가
80% 였다고 합니다.
저자들은 만약 환자가 패혈증이 동반되어 있다면
한번의 세척 조치(I&D)로 해결할 생각을 하지 말고
여러번의 세척 조치(I&D) 및 추가 조치를 할 각오를 가지라고 말하고 있네요.
One factor that can prevent a solution with a single irrigation&drainage when an infection occurs after spine surgery :
In the April issue of the renowned academic journal The Spine Journal, an analysis was conducted on the causes of failure in dealing with infections that occur after spinal surgery.
This study was conducted as a retrospective study at Yale University in the United States.
The infection rate after spinal surgery has been reported to be around 1-3.1% in previous studies.
The standard approach in case of infection after spinal surgery is to reopen the wound, clean the wound area, insert a drainage tube, and use intravenous antibiotics.
Researchers found that in 72.7% of cases, a single cleaning resolved the infection.
However, in 27.3% of cases, a single irrigation&drainage did not resolve the infection.
A crucial factor in the failure of infection resolution with single irrigation&drainage was the presence of bacteremia in patients.
Bacteremia is a condition in which bacteria are detected in blood tests. In cases with bacteremia, 80% of surgical site infections were not resolved with a single irrigation&drainage.
The authors suggest that if a patient has bacteremia, be prepared to take multiple interventions instead of expecting to resolve the infection with just single irrigation&drainage.
