Domperidone is a medication primarily used to treat nausea and vomiting, as well as to relieve symptoms of gastroparesis (delayed gastric emptying) where it enhances gastrointestinal motility. Unlike many other antiemetic medications, domperidone does not readily cross the blood-brain barrier, which reduces the risk of central nervous system side effects. It works by blocking dopamine receptors in the gut and the area of the brain that controls vomiting, thus improving symptoms of nausea and promoting gastric emptying.
Mechanism of Action
Domperidone acts by antagonizing dopamine receptors, primarily in the gastrointestinal tract and the chemoreceptor trigger zone (CTZ) in the brain, which is outside the blood-brain barrier. This action facilitates gastric emptying and suppresses the vomiting reflex.
Common Uses
Nausea and Vomiting: Effective in treating symptoms caused by a variety of conditions, including gastroesophageal reflux disease (GERD) and chemotherapy.
Gastroparesis: Helps in managing gastroparesis by improving the movement of the stomach and intestines, allowing food to move more easily through the stomach.
Dosage Instructions
The dosage of domperidone can vary depending on the condition being treated. Typically, for adults, the dose is 10 mg taken three to four times a day, 15 to 30 minutes before meals and, if necessary, before bedtime. Always follow the healthcare provider's instructions and do not adjust the dose without consulting them.
Benefits
Provides effective relief from nausea and vomiting.
Improves gastrointestinal motility, beneficial for gastroparesis patients.
Low risk of central nervous system side effects due to minimal blood-brain barrier penetration.
Potential Side Effects
While domperidone is generally well-tolerated, some individuals may experience side effects, including:
Headaches.
Dry mouth.
Abdominal cramps.
Dizziness.
Rarely, it can lead to more serious effects like heart rhythm changes, especially in higher doses or in patients with pre-existing heart conditions.
